U.S. Customs and Border Protection initiative commonly known as ‘Customs – Trade Partnership Against Terrorism’ or the C-TPAT Program, is designed to offer partnership with the cooperation of the private industry in efforts to enhance the security of the international supply chain.
As a part of our commitment, Horizon International Cargo Inc. must assess the security practices of all our business partners and service providers.

A questionnaire is enclosed for your completion to the best of your ability. This questionnaire will serve as a tool for that assessment.
It is our expectation that our business partners cooperate with C-TPAT Program to strengthen the security of the global supply chain.
Upon completion of the attached questionnaire you will be asked to log your name and email address, then asked to click submit. This will then submit your details electronically to us. If you wish to keep a copy of the form please print the Adobe file before you submit.
